STUDENT ACCESSIBILITY SERVICES

Qualified students are encouraged to disclose known disabilities and to request reasonable accommodation as early as possible. Students who do not disclose disabilities and request  accommodation until they have encountered academic difficulty may be jeopardizing their chances of successfully completing the program. Students needing more information regarding the Americans with Disabilities Act policy or procedures should contact the Assistant Director of Learning Center and Accessibility Services.

Submit Consideration Request

To submit accessibility requests for consideration, please complete the following steps:

Step 2

Complete the Student Request for Accommodations Intake Form located on the NEOMED Maxient Portal.

Step 3

Submit medical documentation using the provider form to verify the disability and its impact. You may also submit additional documentation through Student Accessibility Services Documentation Upload Link such as prior IEP/504 Plans, psychoeducational evaluations, or records of previous accommodations.

Step 4

You will receive a letter via email stating your request is under review. At this time please schedule an intake meeting with the Accessibility Specialist as part of the interactive process.

Step 5

Accommodation decisions are made through an individualized, interactive process. Most requests are reviewed within 2–3 weeks once all documentation from the interactive process has been reviewed.

NATIONAL TESTING ACCOMMODATIONS

Visit your college’s licensure website for information on requesting national testing accommodations. Be sure to review all deadlines and documentation requirements carefully.

Please reach out to the Accessibility Specialist if you are in need of support with applying for national testing accommodations.

Please note: Students should pay special attention to the timelines and requirements of these national testing accommodation requests.
