Federal Transparency in coverage – Machine-Readable Pricing Files

The machine-readable files that are made available in response to the federal Transparency in Coverage Rule and include negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers.

Machine-readable files are formatted to allow researchers, regulators, and application developers to access and analyze date more easily. Learn more about the machine-readable files.

Enrollment and Eligibility

Full-time employees working 40 hours per week are eligible for medical insurance and all other benefits. Employees working 30 hours per week are eligible for medical and prescription coverage only.

Eligible dependents include spouse and dependent children to age 26.

Working spouses who have access to employer-sponsored medical coverage will not be eligible to enroll in NEOMED’s medical and prescription coverage. Spouse is eligible for dental and vision coverage.

How to Enroll in Coverage

New hires are eligible for coverage beginning the first of the month following the hire date, unless the hire date falls on the 1st of the month, in which case benefits start immediately.

Employees are eligible to enroll in NEOMED health coverage within 30 days of a qualified life event, such as marriage, divorce, the birth of a baby, or job loss.

Open Enrollment

The opportunity to add or delete coverage occurs during Open Enrollment during the month of November for an effective date of the following plan year.

Medical

The university offers medical coverage through Medical Mutual of Ohio that includes a PPO (Preferred Provider Organization) Plan and two High-Deductible Health Plans.

The PPO plan has a $25 copay for primary care visits and a $30 copay for specialist visits, with an $800 individual deductible and a $1,600 family deductible.

The in-network out-of-pocket maximum is $5,000 for an individual and $10,000 for a family. In contrast, the HDHP options offer lower monthly premiums but require higher out-of-pocket costs before coverage begins.

The Higher Deductible Plan has a $5,000 individual deductible and a $10,000 family deductible, while the Lower Deductible Plan has a $3,400 individual deductible and $6,800 family deductible. The HDHP plans cover annual preventive care at 100% and allow participants to contribute to a Health Savings Account (HSA), providing a tax-advantaged way to save for medical expenses.

Pharmacy benefits are through Express Scripts and are included in the medical plans.  A representative is available to assist with finding an in-network pharmacy, determining the cost of your medication, mail order option, checking the status of mail order claims and other prescription drug questions.

Health Savings Account

A financial aid specialist talks with a student at an outdoor campus event.A Health Savings Account (HSA) administered through Health Equity is a savings account designed for employees enrolled in a High-Deductible Health Plan (HDHP). It allows you to set aside money on a pre-tax basis and use those funds tax-free for qualified medical expenses. HSA funds can also be invested in a variety of investment options, and any earnings grow tax-free. Unused funds roll over from year to year, so you never lose your balance, and the account is yours to keep even if you change jobs or insurance plans. For 2026, the annual contribution limits are $4,400 for individual coverage and $8,750 for family coverage, with an additional $1,000 catch-up contribution allowed for individuals age 55 and older.

Flexible Spending Accounts

Flexible Spending Accounts (FSA) administered through Health Equity are employer-sponsored benefits that allow you to set aside pre-tax dollars from your paycheck to pay for eligible healthcare or dependent care expenses during the plan year. For 2026, the contribution limit is $3,400 for a healthcare FSA and $7,500 for a dependent care FSA. The full annual healthcare FSA election is available to you at the start of the plan year, even though contributions are deducted throughout the year. However, FSAs operate under a “use it or lose it” rule, meaning unused funds may be forfeited at the end of the plan year, although a limited portion of your remaining balance may be rolled over to the next plan year.

Dental

Dental insurance is provided to help employees and their family members maintain good dental health through regular preventive care. The dental plan is administered through Beam Dental. The annual deductible is $50 for an individual and $100 for a family. Preventive care is covered at 100% when using in-network dentists and includes exams, cleanings, X-rays, and sealants. Basic services, such as fillings, simple extractions, and periodontal care, are covered at 80% coinsurance after the deductible is met. Major procedures, including crowns, root canals, and bridges, are covered at 50% coinsurance after the deductible is met. The annual out of pocket limit is $1,250.00 per individual. Orthodontia is available for dependents up to age 19, with a $750 lifetime maximum. To find participating providers, you can visit the Beam Benefits website.

Vision

The vision program is offered through VSP and provides coverage for annual eye exams as well as benefits for either glasses or contact lenses. The eye exam copay is $15, with up to a $39 copay for a retinal screening. The plan includes a $250 featured frame allowance and covers single vision, lined bifocal, and lined trifocal lenses, while lens enhancements require an additional copay. Disability

Unum Short Term Disability insurance provides partial income replacement if you are temporarily unable to work due to illness, injury, accident, or pregnancy.  The plan covers 50% of your weekly income up to 1,200.00, for a period of up to 24 weeks.

Unum Long Term Disability insurance provides on going partial income replacement if you are unable to work for an extended period due to illness, injury, or disability.  There are two plan options available:  The employer paid plan has no cost to the employee and a waiting period of 6 months before benefits begin.  The employee cost-share plan requires employee contributions and has a shorter waiting period of 3 months.  Both plans cover 60% of your monthly income, up to $12,500.

Life Insurance

Life insurance available through Securian Life Insurance Company provides financial protection to your beneficiaries in the event of your death. The policy pays a lumpsum benefit to the individuals you designate, which can be used to cover expenses such as funeral costs, outstanding debts, mortgage payments, or ongoing living expenses. The university provides basic life insurance at no cost to employees, with coverage up to $100,000. Accidental Death and Dismemberment Insurance is equal to the amount of basic life. Supplemental life insurance plans are also available, allowing you to purchase additional coverage for yourself, as well as coverage for your spouse and dependent children. Life insurance offers peace of mind by helping ensure your loved ones have financial support when they need it most.

Identity Theft Protection

Identity Force provides comprehensive identity protection, including mobile device and PC protection, child monitoring, identity restoration services, credit health monitoring, and financial account protection. This program is available to you and your household family members, covering up to ten additional adult family members in your household, as well as offering senior fraud protection. Plans also include access to credit monitoring, fraud alerts, and insurance coverage for certain expenses related to identity restoration, helping you stay protected and recover more quickly if your personal information is compromised.

Employee Assistance Program

The university partners with All One Health as an additional wellness benefit program providing counseling, legal assistance, and more for employees and their household. Free, confidential telephone counseling service for employees and their family members for up to three sessions per occurrence. Retirement Options

State of Ohio public employees do not contribute to social security and in lieu of contribute to OPERS, STRS (faculty) or an Alternative Retirement System (ARP 401(a)).

Supplemental retirement options are available through a 403(b) plan and/or a 457(b) plan. These employer-sponsored retirement savings options allow you to set aside money on a pre-tax or Roth (after-tax) basis, helping you save for retirement while reducing your taxable income. The university vendors include, TIAA, VOYA, AIG/Valic, Equitable and Deferred Compensation.

Pet Insurance

Pet insurance through Pet Partners provides Accident and Illness coverage for your cat or dog, although pets aged 11 years or older are not eligible for coverage. The plan includes a $250 annual deductible, 80% coinsurance, and a $5,000 annual benefit limit. Accident-only coverage includes emergency care and prescription medications related to covered incidents such as auto accidents, animal bites, and dental trauma. The Accident and Illness coverage option also includes treatment for illnesses, sickness, disease, or any change in your pet’s normal, healthy state that is not caused by injury.

Family and Medical Leave Act (FMLA)

Eligible employees may take protected leave when they need time to care for their own serious health condition or to care for an immediate family member. Read NEOMED’s policy about Family and Medical Leave Act (FMLA)

Qualifying Events

Certain life events may allow you to enroll in benefits or make changes outside the regular enrollment period. These include losing coverage through another employer or removing a dependent who becomes eligible for benefits through their own workplace.

Educational Benefits

After six months of continuous employment, university employees may qualify for reimbursement of college instructional fees at NEOMED’s partner universities for themselves or eligible dependents. This benefit is available during the first enrollment period after the six-month employment milestone. More about educational benefits at NEOMED.

Vacation Leave

Eligible University employees may accrue and use vacation leave in accordance with University policy to support work-life balance and personal time away from work. Read about vacation leave.

Sick, Personal and Bereavement Leave

University employees are eligible for sick, personal, and bereavement leave in accordance with University policy and applicable Ohio law.
